New Patients

All new patients should arrive 10-15 minutes prior to their therapy appointment.  For your convenience you can download most of the forms from our web site.  Click on Patient Forms. 

It is important for you to bring with you on your first appointment the following,

• Prescription from your referring physician
• Your referral, if needed
• Insurance card
• Picture i.d.

Please dress comfortable for your therapy appointment.  Most patients will wear or bring shorts, sweats, t-shirt, and tennis shoes. We have private treatment rooms to change for your convenience.
